Healthcare & Disability Services
How an Australian disability services provider replaced paper-based operations with governed digital workflows and real-time clinical health monitoring, configured by their own team, without writing a single line of code.
Paper Forms, Scattered Systems, Rising Risk
A mid-sized disability services provider supporting clients across multiple residential sites needed to replace paper-based incident reporting, compliance tracking, and health monitoring with a governed digital system, without a development team.
Client safety at scale
Support workers across multiple residential sites needed to record incidents, hazards, and health observations, but paper forms meant delayed reporting and no real-time visibility for managers.
Regulatory compliance obligations
Industry regulations demanded documented incident management, hazard reporting, complaint handling, and audit trails: all with proper escalation and sign-off workflows.
No clinical health monitoring
Client vitals and clinical observations were recorded on paper with no automated risk detection. Abnormal readings could go unnoticed for hours.
No development team
With all staff focused on care delivery, there was no internal IT team or budget for custom software development. Any solution needed to be manageable by operations staff.
From First Form to Clinical Monitoring
The organisation started with a single incident form and grew organically: each new form solving a real operational problem, configured by their own staff.
Replacing Paper-Based Incident Reporting
The first priority was the highest-risk process: incident reporting. Paper forms meant delayed escalation, inconsistent documentation, and no audit trail. BPA replaced this with digital forms, multi-stage approval workflows, and automatic email notifications routed to the right department, giving managers real-time visibility across all sites.
Expanding Across the Organisation
With incident reporting running, the organisation's own staff configured new forms across WHS, HR, compliance, finance, operations, and residential workflows. Each form was iterated dozens of times as real-world feedback shaped the design: no developers involved.
Clinical Health Monitoring
The organisation then deployed a purpose-built clinical observation app for real-time recording of standard vitals and observations. Every reading is automatically evaluated against per-client thresholds, with tiered email and SMS alerts to managers and executives when readings fall outside safe ranges. Two dedicated interfaces: one optimised for support workers recording observations, one for administrators managing risk.
Seven Business Domains, One Platform
Every domain was digitised as configuration, forms, workflows, notifications, and audit trails, by the organisation's own operations staff.
| Domain | What Was Paper | What BPA Delivers |
|---|---|---|
| WHS & Safety | Manual paper-based reporting with delayed escalation and inconsistent records | Digital incident and hazard forms with multi-stage approval, automatic escalation, and full audit trail |
| Operations | Paper-based daily logs and informal event tracking with no central record | Structured operations logs and event tracking with automatic notifications to the right teams |
| Compliance | Inconsistent logging with no documented resolution workflow | Governed feedback and resolution process with sign-off and audit history |
| HR | Manual requests and queries handled by email with no tracking | Self-service forms with approval workflows, status visibility, and automatic notifications |
| Finance | Requests handled by email with no approval chain or visibility | Governed multi-stage workflow with review, rejection handling, and document management |
| Residential | Informal requests with no record of what was raised or actioned | Structured forms with approval, notification, and history |
| Health Monitoring | Paper-based vitals recording with abnormal readings often noticed too late | Real-time observation recording with automated risk evaluation, per-client thresholds, and tiered email/SMS alerts |
From Paper Vitals to Automated Risk Alerts
The most impactful addition: a clinical health monitoring system that turns every vital reading into an automated safety check, configured entirely on BPA.
What Support Workers See
A streamlined app to record standard vitals, health assessments, and routine clinical observations. Auto-timestamps, auto-attribution, one-tap submission.
What Happens Automatically
Every reading is evaluated against per-client thresholds. If a reading falls outside safe ranges, tiered alerts fire immediately, email for moderate risk, email + SMS for emergencies, to the right managers based on site and severity.
What Managers See
A real-time dashboard with risk distribution, alert history, and recording trends. Full visibility across all sites: no more waiting for paper to arrive or phoning around to check on readings.
What Administrators Control
Per-client threshold customisation: adjust safe ranges for individual health profiles, suppress alerts for known conditions, or escalate specific indicators. Every override is logged with a reason for audit purposes.
"We started with a single incident form. Today we're running governed workflows across seven business domains, plus a clinical health monitoring system with automated risk alerts: all configured by our own team, without writing a single line of code."

What Was Delivered
Paper eliminated across every domain
Seven business domains, from incident reporting to health monitoring, moved from paper to governed digital workflows. Every process now has structured data, automatic routing, and a complete audit trail.
Abnormal readings caught in real time
Vital readings that would have sat on paper for hours now trigger automated risk alerts within seconds. Managers and executives are notified by email or SMS based on severity: no manual escalation required.
Staff adopted it because it helped them
No mandated rollout or formal training programme. Usage grew organically as staff across every department found the platform genuinely useful for their daily work.
Operations staff own the system
Forms are iterated directly by the people who use them, refined dozens of times in response to real-world feedback. No developer backlog. No IT tickets. Changes go live the same day.
Right person, right time, every time
Every submission triggers the correct approval workflow, routed to the right department group with templated email notifications. No submissions lost, no approvals missed.
Audit-ready from day one
Every submission, approval, rejection, and field change logged with timestamp, user, and context. Complaint resolution and incident escalation documented end-to-end: ready for any regulatory review.
What Made This Possible
Form Designer
Drag-and-drop form builder with conditional fields, multi-stage workflows, and email notifications. Operations staff iterate forms directly: no developer needed.
Approval Workflows
Multi-stage approval chains with group-based routing, rejection handling, and task assignment. Department-based notification groups ensure submissions reach the right people.
Automated Risk Engine
Process-based risk evaluation compares vitals against per-client thresholds and fires tiered alerts: email for medium/high risk, SMS for emergencies.
Purpose-Built Apps
Two dedicated apps for different audiences: a streamlined recorder for support workers and a full admin dashboard with analytics, alert history, and configuration.
Notification Overrides
Per-client customisation: suppress alerts for known conditions, adjust thresholds for individual health profiles, or escalate specific indicators to SMS.
Multi-Tenant Isolation
All data scoped to the organisation. Site-based groups control visibility. Infrastructure delivered by Norbrik, an ISO 27001 certified company, with a full audit trail.
