Put a form on your own website

Two ways to get a form in front of people who do not use BPA.

Before you start

  • A published form.

Steps

  1. Open the form and choose Share.
    You will see a link, and an embed option.
  2. To share a link, copy it and send it.
    Good for email, a newsletter, or a QR code on a poster.
  3. To put it in a page, copy the embed snippet.
    Paste it into your website where you want the form to appear.
  4. Open the page and fill the form in yourself.
    Always test from outside BPA, ideally signed out or in a private window.

What you should see

The form appears inside your page, styled to match, and a submission lands in Submissions like any other.

If it does not work

The form area is blank on my site.
The form is probably not published, or the page is cached. Publish, then reload with a hard refresh.

It submits but I see nothing in Submissions.
Check you are looking at the right form, and clear any date filter.
